Student-Athlete Forms: 2009-10

All Cabrini student-athletes are required to complete the forms listed below before participating in any athletic activity.

All Student-Athletes
These compliance forms need to be printed, completed and brought to the mandatory compliance meeting for all winter and spring sports Thursday, September 17 from 7:30 p.m. - 8:30 p.m. in the Widener Lecture Hall. (Forms must be submitted in proper order No. 1 - 5):

2. HIPAA/Buckley Amendment Consent/Waiver Form
3. Student-Athlete Statement
4. Drug-Testing Consent Form - Championships Segment
5. Summary of Regulations

All Freshmen and First-Year Student-Athletes/Transfers
Come to the training room on the designated physical date with the following forms:

Preparticipation Physical Evaluation - Medical History (Print BOTH pages, but fill out Page 1 ONLY)
Emergency Contact Form
HIPAA Form
A photocopy of health insurance card

NOTE: The freshman physical required to enroll in school is DIFFERENT from the medical evaluation. An additional physical must be completed by the Cabrini College team physician before a student-athlete can participate in any athletic activity. 

Returning Student-Athletes (Sophomores, Juniors and Seniors)
Come to the training room on the designated physical date with the following forms:

Emergency Contact Form
HIPAA Form
A photocopy of health insurance card

Student-Athletes Returning From Injury
If you are a returning Cabrini College student-athlete that had an injury which required surgical repair and/or had to undergo extended physical therapy/rehabilitation from a medical professional (physical therapist or chiropractor), you will have to go through the entire pre-participation physical examination with the Cabrini College team physician and you will need to bring the following items in addition to the necessary materials noted above:

• If surgery was required: a signed written note from the surgeon stating if you are cleared for full participation or if you have any restrictions; if special bracing is required, that should also be included on the note.

• If you underwent extended physical therapy/rehabilitation with a medical professional: any notes clearing you to return to participation and a corresponding note from a medical physician (M.D. or D.O.) stating the same.
